It has pointy hands, not straight cut like most of these variations.

I wouldn't call it a model. It's a 145.016. These watches weren't produced in thousands of examples so it can well be that some of them were assembled with what they had in their parts at that time. Wouldn't be the first reference where such things can be observed.
But it's def interesting to log that dial / hands combination here 👍

Well that’s happening for sure. Do those pics come with it once it comes back? I’ve never seen omega do that stuff!
Your dial looks incredible. Mine has some issues around the 11 o’clock marker, wonder if a replacement dial would be NOS

Looks like the restoration shown by @rhino7 above would have involved dial restoration (assuming anything was wrong with the dial) rather than a service dial, give the tritium markers.
This raises a question I've been wondering about for ages - does Omega actually make service dials for the 145.006 / 145.016? Has anyone seen one out in the wild or on a parts list?

It was full restoration service and took about 6 months. Here's a photo of all parts replaced. One of the hands was actually bent. Cost was around $1100 AUD.
Very happy with the outcome.

No dials are currently available for those cases, so any dial that was sent in that was deemed unacceptable by Omega, would be refinished.
