Indeed - I called my local ob and they emailed me the form, which I emailed to Swatch.
I ordered an extract in October, which has not yet arrived - I checked in around the first of the year and they replied that my request had been lost or something. The plus side is they're waiving the fee, so I don't mind after all!
Hi I ordered an extract of the archives yesterday at my local boutique (3592.50.00) and was told they can take 5-6 months to be delivered. What has been the experience by other members? Think read somewhere they usually take about 4-6 weeks
Every extract is signed by the CEO and I think, he has more to do than just signing extracts. 馃榿 So it can take some more time if he is on a business trip or on vacation (yes, he is doing vacation as well 馃槈). I think that is the reason it it takes sometimes 4 weeks and sometimes up to 8 weeks.

Well today got a reply to my request inquiry on the status of the extract of the archives for my 3592.50 numbered edition (below); not available 馃檨. Another OF member had the same faith with his request for his numbered 3592.50, so we share the same faith
got mine in about 8-9 weeks. it takes time but it will eventually arrive. if you order via their website, there is a part on their website where you can view the status.
Well mine arrived today, in a nice posh red envelope, after eight and a half weeks.馃憤
It confirmed that the, 24531xxx, 321, movement was indeed correct for my 105.012-65 Speedmaster and that the watch was delivered to Gibraltar in January 1967. (I've added it to the original post of the watch in the link below)
