It's probably not something that affects every single watch, or else I have no idea how it got past the development stages of the 3861.
That plus the consideration of how many people with the Apollo 11 series just keep it in a safe, and how many just dont frequent watch forums. Theres probably a few affected Apollo 11s out there that we'll just never hear about.
Got my Apollo 11 50th in Aug 2019 and have wound and played with the chrono countless times to see if this issue is going to occur on my example. But it's rock steady with nary an issue yet. Still have 2 1/2 years left on the warranty so plenty of time for the issue to occur, but perhaps it won't.
This is such a huge thread, and perhaps it’s been asked and answered somewhere before, but I’m super curious if Omega has ever issued a recall before?
Would this issue warrant one?

I've worn my new Speedmaster for about 4 months (daily) and then it stopped, and it seems it was caused by this issue. It took almost 4 months but Omega has now returned the watch to me, including the replaced parts. I was a little bit shocked when I saw the center wheel, either something strange happened inside the watch, or the wheel was removed with a lot of force. Any ideas what could have happened? And why not only the center wheel was replaced but also two others wheels (and also the mainspring by the way..).

The bent wheel is damage done after it was removed, and not the problem that caused your watch to stop. No watch would run at all with a wheel that looked like that. Possible it was dropped on the floor and stepped on or something.
