I'm confused

With the reference 22258 shouldn't be stainless steel instead of gold plated? My 22258 is stainless steel and, if i remember well, ELV web specimen is ss too.

On the other hand the shape of case and lugs seems similar to mine

Seems clear to me that watch has been re-plated but that only make sense in previous gold plated watch

Is the case back from a different watch?

It's possible that references 2xxxx was also used on gold plated watches instead of references 4xxxx?

It is either a Franken (put together with different case back and case) or the owner went and plate the steel (I am not sure if doable or not).

The first digit of Universal model numbers indicates the case material: 1 is 18kt gold, 2 is steel, 3 is chrome plated, 4 is plated, 5 is 14kt...

So my guess is that it is a franken with changed back.
