Omega Naïad - Medicus

I believe I have just acquired an Omega Medicus. Has anyone seen a dial similar to this? I have seen similar layouts but the dial material/coloring is what I find interesting about this specific watch. What do the numbers that are engraved on the bottom of the lugs represent? I have seen many of these models with various numbers.

Is this a ref 2064?

The crystal on the watch spins freely, can someone assist in sourcing a proper replacement crystal?

The number on the rear of the lugs is a case serial number. It will match the serial number on the inside of the caseback.

The oversize caseback is a dead give-away, I wish I had better pictures of mine, I'll add it to my "to do" list as these are cool, historical pieces.
We have a Mentor here, @Tire-comedon, that is a world-class authority on these, perhaps he'll see this.

I think it is a CK 2064 Naiad

Here is mine 😀
As you probably know, you are missing the spring that pushes against the second hand pinion shaft on your movement.
