If it's an Omega you're looking to service, you have a Swatch Group USA (Omega) Service Center in town. If you are interested in using them, send me a DM and I can send you the forms they use when you submit a watch. Or you can send an email to Omega and ask for it if you would prefer.

Instead of using a middle man or the incredibly slow email route, the OP can just call the Swatch service line and they will provide him the paperwork while on the phone. Though, this is assuming the OP has a modern watch that falls under the Swatch umbrella. If it’s vintage and the OP wants to preserve it, they should find an independent to do the work. With the limited info the OP has provided, it’s anyone’s guess what exactly they need.
