    Potentially a quick question for the experts, was the Pulsations bezel ever a factory option on a 1969 145.022?

    Yes, because some catalogs from the 80's still refer to optional bezels

    Note the line under the numbers but over the indices, like in the (first) photo. Later replacement pulsation bezels don't have that line.

    The pulsation bezel is actually great for medics, paramedics and sportsman. !!!

    Yes, as Time Exposure says, be warned that there are original vintage bezels and modern replacements. For a -69, you obviously want the proper vintage one.
