Omega 321 problem

Working on a 321 movement that came with no stem, I replaced the stem Crown and set lever with genuine NOS parts and the stem just pulls out of the movement. I have tried 3 stems and 2 set levers all NOS and same result. Any advice. Seems the lever is not engaging the stem even though it is fully tightened.

Would need to see some macro closeup photos of the installed setting lever & stem that shows the interaction between the parts.

Is there any wear in the hole in the main plate and barrel bridge that the stem goes through? If that is worn oversize, it can allow the stem to be pulled out.
