Can anyone tell me if the bezel insert is missing or just completely faded away? The pearl pip is still attached and I see absolutely no numbers or trace of any color where the insert should be so I’m very confused.

Insert is still there. I’m not sure I’d call it faded away. Looks like it is more likely sanded/polished away.
Due to the layering in the anodizing process for Rolex/Tudor inserts, the color would fade first, then eventually the numbers. You’ll notice even ghost bezels still show the numbers even if the color is gone.
In this case, with everything gone, it seems highly unlikely the bezel would have gone thru such extreme fading to wear out the numbers too.

Crazy, why would the original owner do this to the bezel? Asking because I recently bought it off ebay and currently in shop. Wondering if I should put a new old bezel insert in. Original bezel color should have been blue.

Also, found two possible replacements for it. How can I tell if they're fake. Here's pics of both. One looks to be in better condition, but missing pearl. It went for $150 while the other has aged pearl, looks pretty authentic, and went for $400

I’m not familiar enough with this reference to give tips about authenticity of the parts. But generally speaking:
- Research trusted sources (for me, VRF/VRM and HQ Milton) for their pictures. Compare, compare, compare. Look at all the little details like number positioning, serifs, color, texture, pip placement, etc.
- Research the seller and buy the seller
- Ask the seller questions about it and flat out ask if it’s genuine
- Post pics on VRF and ask for feedback
Personally I would replace the bezel.
Good luck!

Here’s my slightly earlier 76100 - and what a faded bezel should look like... problem is the genuine ones also fit Rolex subs of the era, so the nice ones have gone up in price...
