Help Identifying Constellation

Hello,

My father passed down an Omega Constellation and I'm trying to identify the model.
From my research on the vintages site I think it's the 168.0005 or the 168.0065. However, in both cases there are some notable differences:
ST 168.0005 is the closest match, I believe, but it is missing the date function
CD 168.0005 has the date function, but the minute indicator dots are not in the same location as mine
ST 168.0065 same as above, close but minute dots are different

I didn't attach pics of the above models so as not to confuse them from mine.

Everything is inside on the case back, open it then you can see.

It’s either a 14902 or more likely a 168.005, - definitely not a 168.065.
As above inside the screwed case back will reveal all the numbers.
ST simply means steel and CD yellow gold capped.
