We have covered this before. Omega does not have trademark rights to sell/do business, in the US over the internet. They can not provide this service to US customers electronically. You can get the form and order an extract from Switzerland via mail by going a local boutique in person.
You can request the forms and order one through Omega US customer service. I have done it quite a few times, most recently in November, 2014. Payment is processed and extract mailed from Bienne. [email protected]

They do Not sell watches or service over the internet, nor do business. This is not just due to business practice ( though with watches, no doubt it is) but due to trademark restrictions (not prohibitions) with the word, Omega in the US. This comes direct from Bienne at the highest levels. If you look at the online form you can see that structurally there is nothing that would prevent them from doing it as there would be no difference between the US or anywhere else. It is all legal and attorneys. I was told in no uncertain terms, "It is not us."
Actually, I believe that the ban on any internet sales at all is part of the agreement between Omega and their US dealers, including the boutiques. gatorcpa
Seems foolish to restrict the manufacture from supporting the customer...why wouldn't they give an exception to support related online transactions. Ive sent an email to the address above, so far no reply.
There is a legal loophole that would allow them to make an Extract available over the internet in the US...They could make it free of charge like Longines.
