Welcome to the forum. After a bit of research, it appears to be a ref. 1212.30.00, steel and 18ct yellow gold, quartz cal. 1532. Discontinued when the Constellation range was overhauled in '09
Its a very unique design the modern constellation, 100% original to Omega, and made now for about 30 years. In Asia its more recognizable than some Rolex models. Former Australian Prime Minister Kevin Rudd wears one daily in two-tone identical to the one in your picture:

Now that you mention it the crown does look wrong, and the Omega logo is a different size from the 1212.30.00 compared to the size of the dial. Your best bet is to take it to an Omega dealer and get them to check it out.
