From what point to what point are the measurements taken on AT watches?

41.5 or 38.5 is it the bezel outter ring? Or case to case from 10 o'clock to 4 o'clock?

It'll be the diagonal of mid case just above or below the crown ie not the bezel or crown or crown guards. Your 10-4 suggestion kind of covers it.
