I need help figuring out what model and time period this watch is from so please help me.

Looks like a generic Omega auto from 1966ish or so. Reference number is 165.0020 as it says on the case back. Appears to be a nice original dial, original crown. May be a franken, should have a cal 552 in it I believe (could very well be wrong). Will look nice after you have the crystal replaced.

1966. What does the other side of the case-back look like. This case reference (165.002) was used for Seamasters and Geneves and sometimes with both indicators.

Reference is 165.002 as you can see on the back of the caseback (I would say seamaster or geneve, but not 100% sure as there is nothing on the dial)
550 Cal movement
Going by serial number, 1966/67 or thereabout

Thats my uneducated guess.

You've got a movement serial number in the 23 millions, so consulting this chart http://chronomaddox.com/omega_serial_numbers.html (which I found by googling "omega serial number chart") we find it was produced about 1966, give or take a year or two.
Only the higher end Omegas had named models, this did not.

Looking at the pics again, it appears the the bracelet that is on the watch is ill fitting. If this is true, you should remove it before it causes damage to the case.

Seconded. Poorly fitting bracelets can gouge out and damage the lugs.

Caliber 550 is fine for this case reference.

Looks like a redial with Seamaster omitted. Should be 552 movement. Reference below.

There are many dial variances seen with this reference and the OP's appears original and in good shape. Cal. 550 has also been seen on this ref. The 165.037 replacing this 165.002 and seems to still have cal. 550.

That's an interesting effect, the paint flaking off by the hour markers. I wonder if the tritium reacted in some way with the paint?

This reference has Many dial variations.
