Because these boxes are silver and can be used for many things, they often survive even the watches that came in them. Unfortnately the utility of the boxes means that almost invariably the inserts were discarded. Here is a box owned by Doug Gravina with the original insert. And here is a box I own where I made a new replacement insert. Another box, this one for a Constellation in an Omega ad.

Miki I can make you an insert for one of those which is extremely close to the original, a few people on here will know that I have a bit of a 'geek' trait in me and it should't be an issue to replicate the original, first because I have 6 NOS versions of the original with their inserts and secondly because I made insert for the three others I have that had them missing
I have a bit of a thing for collecting Omega boxes to go with my watches, when you have 10 Constellation Megaquartz 2.4Mhz's you need something to keep them in
I don't think Omega ever gave those boxes away. They charged Sfr.100 to 140, or about $25 to $35 for the silver box in 1959. Equivalent of about $700 to $1,000 today, between inflation and devaluation of the dollar. gatorcpa
