I would appreciate any help in the identification of this particular Omega watch model.

It's a converted pocket watch, probably dating to about 1914 (movement serial number), case serial number is about the same date.
It has been converted by welding lugs to the original case so it could be worn on the wrist. This was often done in the 1920s/1930s to re-purpose pocket watches so that thay could still be worn.

However, based on the lug style, and the fanciful dial, I would say this is a recent conversion, similar to many "re-purposed" pocket watches from Ukraine and Central Europe. The dial bears no resemblance whatsoever to anything that left the Omega factory. It also appears to be missing one of the hands (hour? minute?).

It's a style known as a "marriage watch", presumably marrying the old to the new.
