Not sure what you're suggesting (e.g. that I fabricated that form?), but it is well known you can also order an extract by email as I did. If you don't believe me, look at this thread. https://omegaforums.net/threads/omega-extract-of-the-archives.22962/

Not implying anything so no need to get defensive. It's odd that there are mixed messages from Omega. I am aware of that thread and still wonder why the website says you can't order on line when you actually can.
Maybe it's semantics about what is meant by "online". You can't submit your request directly through the website, which you can do outside the US. But you can send them the form by snail mail or email.
Last time I ordered an extract I also submitted a support request in order to ensure that the serial number of the watch matches the watch reference. The extract took about 6 weeks and the support case about 4 days. Don‘t know if this helps
I emailed Swatch USA and was sent the PDF form that @Dan S showed. Took about 10 weeks until it was mailed to me.
Did the same thing and waited approximately 8 weeks to receive the Archive in the mail. Before delivery, Swatch USA will email a confirmation. I find this most convenient if you are based in the US.
If the request is urgent to be shure simply call,the museum from Tuesday to Friday give the serial number and they will tell you, at least you will be fixed , and ask the archive later .
There's some inconsistency with Swatch USA I'm sure. It took about three weeks for them to acknowledge my initial email request for an extract.
I sent a message to Omega regarding 2 speedmasters that I own. They returned my email with some forms to fill. I sent them back with my credit card information but never heard back from them again, despite the 2 or 3 messages that I sent afterwards.
